Only what the companies need to find your records. Put each item on its own line.

Include any middle names.
Never neededDate of birth, National Insurance number, bank details or ID documents. If a company asks for ID, you can say no: a reply from the same email address shows the request is yours.

Open electoral register. Your council sells this copy of the register to anyone, and much broker data starts here. Opting out does not affect voting or credit checks.
Find your council at gov.uk/find-local-council, then send its electoral services team this:

After you send

One month on: companies must reply within a month. Send this chase-up to any that have not, from the same email thread.
5.2
Still nothing? Complain to the Information Commissioner's Office for free at ico.org.uk/make-a-complaint with copies of both emails.
5.3
Every three months: send the requests again. Companies refresh their data and often add people back.
